Services offered: School programs which are developmentally and age appropriate for infants and toddlers aged 0-3 and their families, services can be provided weekly in the home or in the Center.  Children aged 3-21 attend school daily for 5 hours.  Transportation is provided by the local school district.  Family Support Unit (FSU) programs include the Community Integrated Living Arrangement (CILA) Respite Care and Drop In Respite.  Family Support Units prevent individuals with developmental disabilities from being institutionalized.

Services offered: The DHS Help Me Grow Helpline provides information and statewide referrals in the following areas: adoption, AIDS, alcoholism/drug abuse, child care, child safety seats, child support, dental, disabilities, domestic violence, early intervention, education emergency services, employment, financial aid, family planning, food supplementation, gang prevention, health/physician referral, hearing aids, housing, immunization, infant health, insurance, intergenerational interaction, lead, legal advice, male responsibility, maternity home/school, mental health (general and suicide), miscellaneous, multi-service agencies, nurse midwife, parent/child/teen/mate communication, parenting education/support, parents anonymous, pregnancy/substance abuse, pre-kindergarten, parental, school/peer pressure, seniors, sexual assault, sexuality, sexually transmitted diseases, shelter/drop-in/overnight/transitional, state agency, teratogen (environmental or occupational) and WIC vendors/complaints.  Women’s Health categories include: breast and cervical cancer, cardiovascular disease, depression, domestic violence, menopause and osteoporosis.

Information: Not-for-profit, free-standing community-based hospice.  Licensed by the state of Illinois as a full hospice in 36 counties.  Medicare/Medicaid certified.  Contracts with hospitals and nursing homes throughout service area.
Services offered: A full range of medical and emotional support including physician, nursing, social, chaplain, volunteer and bereavement services that deal with end-of-life issues.
Persons served: Terminally ill patients of all ages and their families.
Fees: None.  Costs may be covered by Medicare, Medicaid or private insurance.

Information: HYFS was established in 1895.  There is a residential unit in Hoyleton, IL for BD/DD children.  The Metro East office was established approximately 20 years ago.
Services offered: 1. Foster care and adoption.  2. Shared cooperative living program: works with CDFS parenting wards in an independent living setting.  3. Youth at Risk: DHHS-funded service for runaway and homeless youth and prevention services for other at-risk youth.  4. Teen parenting services: DHS contract working with parenting teens to facilitate obtaining diploma/GED.  5. Family care services; counseling services provided at location in East St. Louis, Belleville, Fairview Heights, and Edwardsville; Children First in Madison County for divorcing parents; Kids Rebound in Schools for kids who are dealing with divorce/death/grief issues.

Services offered: System of care for children in foster care or relative placement is a multi-service and collaborative planning and delivery of services to stabilize placement of high need children under the guardianship of the State.  Family Support Services are proved to families fro problems of home and child management, child abuse, neglect and/or dependency, licensed and resource utilization.  Youth Seeking Opportunities to Succeed (YSOS) is a community diversion program that provides community based counseling and social services to youth 9-17 years of age who are high risk and/or involved with the juvenile justice system.  Our goal is to provide youth and families with opportunities to develop skills to be successful contributing members of society.  Functional Family Therapy is an evidence based “best practice” family focused model of intervention aimed to strengthen the family and prevent out of home placement.  Multisystemic Therapy also evidence based best practice model, is an intensive family and home based treatment that aims to change antisocial behaviors in multi problem youth and to reduce out of home placement of teenagers.
Persons served: Children under guardianship of DCFS for therapy program.  Homemaker services must be an open family case with DCFS or covered under contract with another agency.  Illinois Counties: Madison, St. Clair, Bond, Monroe, Randolph, Clinton, Washington.  For YSOS, referrals are accepted from Madison County Probation and Court Services, local police departments and the Office of the State’s Attorney.
